In the maternity hospital

Registration of the child begins immediately after birth. Upon discharge from the maternity hospital, the mother of a newborn is issued the following documents:

1. Maternity exchange card

The second sheet (“Information from the maternity hospital, maternity ward of the hospital about the woman in labor”), which contains complete information about the course of labor and the postpartum period and the woman’s health status. It should be taken to your gynecologist.

The third sheet - “Information from the maternity hospital, maternity ward of the hospital about the newborn” - is a report on the condition of the child:

physiological parameters of the newborn: gender, height, weight at birth and at discharge, assessment of the child’s condition; health status of the newborn; the day of the first application to the breast and the day the umbilical cord falls off; carrying out anti-tuberculosis vaccination.

This document is attached to the newborn’s medical record in the clinic where he will be observed in the first years of life.

2. Third birth certificate coupon

You don't need the coupon itself, only the attachments to it. They need to be taken to the children's clinic.

Coupon No. 3 -1 is intended to pay health care institutions for services for the first six months of clinical observation of a newborn. Coupon No. 3 -2 is intended to pay health care institutions for services for the second six months of dispensary observation of the child. 3. Child’s birth certificate

In fact, this is the main document confirming that the child was born by you. It says when and where the child was born, what gender the newborn is, and who delivered the baby. This certificate is valid for 1 month. You need to take it with you to the registry office to issue a birth certificate for the child.

Birth certificate

A child can be registered in any registry office in the city, but most often the child’s birth certificate is issued in the registry office at the place of residence of one of the parents. According to the law, an application for the birth of a child must be submitted no later than one month from the date of his birth.

To register a newborn, you must provide the following documents:

1. The document that serves as the basis for state registration of the birth of a child:

Certificate of birth of a child from the maternity hospital (valid for a month); A birth document issued by a private practitioner during childbirth outside a medical organization; If a newborn was born outside a medical organization and without the participation of doctors - a statement from a person who was present during the birth of the child.

2. Passports of the mother and father (if the family is incomplete - only the mother).

3. Marriage certificate (if available).

When issuing a birth certificate, the surname of the newborn is recorded according to the surname of his parents, the name - by agreement of the parents.

If the parents of a newborn are married, either of them can apply to register the child; the presence of the other when issuing a birth certificate is not necessary. In general, my grandmother went to register my child with the passports of both parents.

If the parents' marriage is not registered, the physical presence of both the father and mother of the newborn is required when registering a birth certificate. Information about the child's father in this case is entered on the basis of the record of the act of establishing paternity.

If paternity has not been established, information about the child’s father is recorded on the birth certificate according to the mother’s words or, at her request, not indicated at all. In this case, the newborn receives the mother’s surname, and the patronymic is written down from her words.

Along with the child’s birth certificate, the registry office will issue a certificate in form No. 25 for processing benefits for a newborn child, which is valid for 6 months. At the same time, the certificate from the maternity hospital is taken away.

If it is impossible to present to the registry office certain documents that are the basis for state registration of the birth of a child, registration is carried out on the basis of a court decision establishing the fact of the birth of a child.

In the maternity hospital

In the first week after the birth of a little citizen, his parents are given a whole stack of papers while still in the maternity hospital. In order not to forget about anything, let’s enter all the documents for the newborn baby received at the maternity hospital into the table:

These are the very first documents for a newborn baby that should be in hand after discharge from the maternity ward.

Main ID

Until the age of 14, the main identification document for a child is a birth certificate.

To register it within a month after the birth of the baby (Article 16 of the Federal Law-143 “On Civil Status Acts”) you need to come to the nearest registry office.

What documents are needed for a newborn child when obtaining a certificate?

The main list is as follows:

  1. The above-mentioned certificate from the maternity hospital.
  2. Passports of parents (or one if a mother or single father is applying).
  3. Certificate of marriage or its dissolution.

Additionally you may need:

  • a statement in Form 6 from a person present at the birth, if it took place outside the maternity hospital;
  • court decision establishing the fact of birth;
  • court ruling declaring the marriage invalid;
  • certificate of death of one of the parents;
  • passport of a personal representative if parents are not able to visit the registry office on their own.

No fees are charged for registration of a children's certificate. At the same time, by the time the documents are submitted, parents must choose a name for the baby and decide whose last name he will bear. It is important to understand that if you do not provide a complete package of papers or if there are errors in the application, the service may be denied.

On a note! An application for a certificate with all the necessary papers can be submitted not only at the registry office, but also at the Multifunctional Center of your city.

In addition to the certificate, which is issued within an hour from the moment of approval and registration of the application, parents receive a certificate of assignment of a one-time benefit. It should be taken to the accounting department at the place of work or transferred to the social security authorities if the parents are not employed.

SNILS

The sooner you correct the pension insurance certificate, the sooner you will be able to start processing the required child benefits.

The following documents should be provided to the Pension Fund for SNILS for a newborn child:

  • applicant's passport;
  • children's birth certificate;
  • a personally written statement based on the Pension Fund model.

SNILS is issued immediately after approval and registration of the application.

After receiving SNILS, you can start receiving benefits:

  • for a one-time payment you will need a passport, a certificate from the registry office issued when registering the certificate, and the certificate itself, a certificate from the place of work from the second parent that no payment was accrued to him;
  • for monthly payments: parents’ passports with copies, certificates for all children, a certificate from the employment center stating that you are not registered, a certificate of family composition, an application;
  • for maternity capital: application, passport, identity cards of all children, SNILS of mother and children.

In the first case, we give the papers to the employer (non-working ones - to the social security department), in the second and third - to the Pension Fund.

Medpolis

A compulsory health insurance policy remains a mandatory document for a newborn child in 2021. It is necessary from the first days of observing the baby in the state clinic. To understand which insurance company to contact, we recommend consulting with the nurse or visiting pediatrician who supervises the newborn.

The insurance company will need:

  • passport of one of the parents;
  • SNILS;
  • birth certificate.

The law allows a month for the preparation of a medical certificate, but so that during this time the baby can use medical services, parents are issued a temporary certificate.

Registration at place of residence

A newborn child can be registered both at the place of residence of the mother and father. At the same time, the consent of all other people registered in this apartment or house is not required to register the child.

To register a newborn, you need to contact the passport office at your place of residence. They will ask for the following documents:

1. Application from the mother (father) to register the child at the place of residence of the mother (father).

2. Extracts from personal accounts and house books from the place of residence of the father and mother (EIRC, passport office).

3. A certificate from the second parent stating that the child is not registered with him or her at the place of residence. (REU, passport office).

4. Child’s birth certificate + photocopy.

5. Parents’ passports + photocopies.

6. Marriage certificate (if registered).

7. Statement from the 2nd parent that he does not object to the child’s registration.

Before submitting to the passport office, all documents, except passports and birth and marriage certificates, must be certified by the head of the housing office. Registration of a child at the place of residence usually takes several days. As a result, a small stamp will appear on the birth certificate confirming the place of residence of the newborn. There is no need to pay any transfers or fees for registering a newborn - it is done free of charge.

If registration takes place at the place of residence of one of the parents, and the other is not registered there, the physical presence and consent of the parent is required for registration with the other parent.

After this, you must receive a certificate from the Housing Office about the child’s registration and about his joint residence with you - this will be needed to receive benefits.

Registration

What documents are needed to register a newborn child?

If the parents live together and are officially registered, then at the passport office you need to provide:

  • their passports;
  • children's birth certificate;
  • marriage certificate;
  • application based on the passport office model.

If the parents do not live together, then the list of papers expands.

To the standard documents required for registering a newborn child, papers are added from the parent in whose living space the baby will not be registered:

  • written consent to registration at the address of the father/mother;
  • a certificate confirming that the child is not registered;
  • an extract from the house register and personal account indicating the lack of registration (required from the father if the baby has not been registered within a month).

Important! You can submit an application for registration on the government services portal, and then at a convenient time, visit your passport office with original documents to complete the registration procedure.

Citizenship

If both parents are Russian, then the baby automatically receives Russian citizenship, and no special actions need to be taken. If the mother or father is a citizen of another country, or the baby was born outside of Russia, then in order to register maternity capital and travel abroad, a stamp of citizenship will have to be affixed.

The following citizenship documents will be required for a newborn child:

  • applicant's passport;
  • children's birth certificate.

A stamp can be placed in the parent’s passport on the day of application:

  • at the passport office of the migration department at the Ministry of Internal Affairs;
  • at the Russian consulate (if the parents are abroad);
  • at the Multifunctional Center.

If the parents do not plan to leave the country, then there is no need to rush to obtain a passport for the baby. If you are planning to move abroad or vacation at an overseas resort, then information about the baby for traveling outside the country can be entered into an old-style adult international passport or a separate document can be sent to the child.

If parents have biometric passports, then children’s data cannot be entered into them.

To obtain a foreign passport for a newborn child up to one year old, you will need the following documents:

  • certificate about the fact of the birth of the baby;
  • parental passports with citizenship stamps;
  • registration certificate with a citizenship stamp;
  • photographs (if required);
  • personal statement of the person submitting the paper;
  • receipt of payment of state duty.

Important! To issue an old passport, you need 2 children's photographs. They are not required for the biometric analogue.

35x45 mm photographs can be taken in a salon or at home, but the photographs must meet the following criteria:

  • have a light background;
  • printing on matte paper;
  • the baby must open his eyes;
  • No head coverings are allowed.

Unlike an adult passport, which is completed according to the standard procedure within a month, a passport for a newborn must be issued within 10 days. You can issue a document:

  • at the passport office, which transfers the papers to the Federal Migration Service;
  • directly to the Federal Migration Service;
  • in the MFC;
  • online on the government services portal.

Stamp in parent's passport

Having figured out what documents a newborn child needs to submit, for many parents the issue of stamping remains unclear. Today, this procedure is optional, but it makes life easier by allowing you to quickly prove that you are the parents of a particular child (if, for example, he is lost in an entertainment center or hypermarket).

To get the stamp, you will need the passport itself and the child’s birth certificate. The stamp is placed on the day of application, regardless of whether you go to the passport office or the MFC.

Important! You can get a stamp at any passport office, not just at the place of registration.

Compulsory health insurance policy

A compulsory medical insurance policy is usually obtained from a children's clinic at your place of residence or from an insurance company working with this clinic. To complete it you will need only two documents:

1. Passport of one of the parents of the newborn, which contains a mark of registration at the place of residence (propiska).

2. Child's birth certificate.

At the time of registration of the policy, you will be given an Application Registration Sheet for the production of a compulsory medical insurance policy, which replaces the policy until it is received. You should also remember that during the first 6 months of life, a newborn is required to provide free medical care, regardless of whether the child has a compulsory medical insurance policy.

If you can only get a policy for a child at your place of residence, then for treatment under the compulsory medical insurance system you have the right to choose any district clinic in the city - even if the child is not registered in that area. Just don’t forget to get an absentee ballot from the clinic at your place of registration - you will need it to receive free medicines and baby food.

Medical policy

So, the new little man has a name and a home, now we need to take care of providing him with medical care. Yes, Article 41 of the Constitution of the Russian Federation guarantees everyone the right to health care and free medical care in state and municipal health care institutions, and emergency medical care is required to be provided even without a compulsory medical insurance policy, but it is still better not to take risks and not to delay the registration of this important document. You can apply for it either at any insurance company or at a local clinic.

For registration you will need:

  • passport of one of the parents;
  • child's birth certificate.

The compulsory medical insurance policy (CHI) gives the right to receive free medical services throughout Russia. Once a year, the patient (or his parents) has the right to choose a medical institution and a local doctor in it.

Registration of citizenship

If before the adoption of the Law “On Citizenship of the Russian Federation” in 2002, parents of a newborn only needed to obtain a birth certificate, registration and compulsory medical insurance, now registration of the child’s citizenship is also required. Without this, you will not only not be able to travel abroad with your baby, but also will not receive a state certificate for maternity capital. In the future, the child will need proof of citizenship to obtain a general passport.

For newborns born after February 2007, the collection of documents for registration of the child’s citizenship is simplified as much as possible. To obtain Russian citizenship, you must submit your parents’ passports to the regional federal migration department (the presence of both parents is not required) and the birth certificate itself. The stamp is affixed directly on the day of application on the back of the birth certificate in the upper left part and in the parents’ passports on the “children” page.

There is no need to issue a separate document on the child’s citizenship when planning a trip abroad.
